Annual 1099-R tax forms for LASERS retirees were mailed on January 29, 2008.
You should receive your form no later than February 8, 2008.
If you have not received your 1099 by February 8, you can request a copy from LASERS by calling 225-922-0600 in the Baton Rouge area; or 800-256-3000 toll free throughout the state.
